It would be good to have a field that allows the enterer to specify where a problem was found. Choices would be "Field/Customer", "Development AdHoc", "Development FT", "Integration Test", "System Test", "Trial/Tech Support". It should be a "no default" field,i.e. user needs to enter it or bug creation will be blocked with an alert.
This would depend on being able to implement custom fields. If that bug was fixed, you could then set the entry templates up in any way you wanted. Making this a required field which couldn't be avoided even by editing the html, and doing so in a generic, customisable, manner, would be interesting, though.
