These failures are still happening on Beta even with bug 1416774 uplifted there. Any thoughts/suggestions?

Reinvestigating. This might be happening a lot more than we realize because we only assert on one code path. My original patch in bug 1383682 added a new related assert in another part of the code, and it tripped a lot. I have a hunch.
