The page at https://www.qualtrics.com/templates/customer-satisfaction-survey/?utm_lp=hub-csat fails to display the "Download Now" form while Basic TP is ON.

This is related to `trackingprotection` breakage #1101005

Looking at the devtools console, here are the blocked resources:

The resource at “https://app-sjp.marketo.com/js/forms2/js/forms2.min.js” was blocked because tracking protection is enabled.[Learn More] customer-satisfaction-survey
The resource at “https://www.googletagmanager.com/gtm.js?id=GTM-W6F8HX” was blocked because tracking protection is enabled.[Learn More] customer-satisfaction-survey
The resource at “https://js-agent.newrelic.com/nr-1071.min.js” was blocked because tracking protection is enabled.[Learn More] customer-satisfaction-survey

So these are the domains to test:

app-sjp.marketo.com
www.googletagmanager.com
js-agent.newrelic.com

I opened the URL in a fresh browser profile (Firefox Nightly 63, uMatrix installed, normal mode) and loaded the page. The page did NOT load completely. Only the "main-nav" bar was displayed.

I disabled the Spoof Referrer option in uMatrix and then *WHITELISTED*:

all the scripts (in order to load the page)
app-sjp.marketo.com

After this, the Download Now form was loaded.
The other resources didn't help.

app-sjp.marketo.com can not be found in any list, but marketo.com be found in disconnect-blacklist.json and disconnect-entitylist.json.

So in conclusion:

The page seems to no longer have the broken form, but given that it is still showing Marketo's forms2.js not loading properly, this bug might as well be duped to bug 1476572.
