1499503 - Provide some way of dismissing the uplift form

There are situations where I don't want to have 3 copies of essentially the same form up or when I'm moving flags incorrectly set for one branch to the correct one. Right now, that leads to a big comment in the bug with an empty form. It would be better if there were a way to dismiss it instead (before, we could just delete the redundant info from the comment box).

Maybe just allowing you to post the form to the comment box before actual submission to allow for editing first?

Bug 1496004 hid the comment box when when the uplift form is inserted... Could we simply add the Dismiss button that allows to discard the form?

Dismiss should maybe open a textarea instead (even if it could be empty)
