replace nsIIOService constructors with CreateInstance

We should replace the constructors that hang off of nsIIOService with
components registered with the component manager instead. These include:
NewAsyncStreamListener
NewAsyncStreamObserver
NewSyncStreamListener
NewLoadGroup
NewInputStreamChannel
