we need a mailto: handler for the no mail-news case

Details
If we're --disable-mailnews, we need to have a default mailto: handler that
directs mailto to a web page or external program.
A useful extension would be the ability to direct mailto: to one of those even
if mailnews is compiled in.
Useful prefs involved: URL/program to send it to (or disable?); send it to
URL/program even if mailnews exists.
I have a (partial currently) implementation I'll post when it's fleshed out.
um. there are plenty of bugs for this.
one is altmail, another is landing protozilla, another is mkaply's bug.
i think for altmail i'm going to refactor mailto: into either netwerk or
extensions, because it really shouldn't need to live in mailnews.

Yeah, this is pretty much a dup of bug 11459. Of course, for all the discussion
there, the only patch (for linux) is mkaply's for external mail apps (not web
uri's). protozilla will do web uri's apparently.
We need to get off our butts and get this dealt with. Major impact to usability
especially in linux.
