[Debugger] Always-visible input field in Watch Expressions and XHR Breakpoints

Steps to reproduce:

Right now if the user has already added at least one Watch Expression or XHR Breakpoint the input field to add a new item is hidden. The best usability seems to be to have it always visible. Once that's accomplished, there's no need to have the plus button (add Watch Expression / add XHR Breakpoint) in the accordion header (top right corner), since when the input field is visible, the plus button just move the focus to the input field. To summarize:

  1. Make the input field in Watch Expression or XHR Breakpoint always visible
  2. Eliminate the plus button in Watch Expression and XHR Breakpoint
