Steps to reproduce:
Clicking HTML links in an email will open a Firefox browser window with the menu bar. If Firefox was already running when the link was clicked, then a new window opens without navigation. If Firefox wasn't already running, then I still get a window and tab without navigation.
Opening Firefox from the Mac dock will often result in a window with no menu bar - just a tab with no navigation.
Actual results:

I assume you're seeing bug 1603956. If that's the case, it should be already fixed in Beta, and coming to release next week.
I'm not completely sure though, because your description doesn't fully match (the problem was only when Firefox was closed and you click an external link, not when it was already open).
