Using the existing Fundraising template as a starting point [see attached], we'd like to create a new snippet template for survey use based on recent requests that have come through.

Requirements:

• Be able to show 2 to 4 buttons
• Buttons should be able to reflect letters or numbers, with or without symbols like $ (what is the maximum character count we could do?)
• Optional to include a checkbox with text [see Fundraising template]
• Editable CTA destination URL (will we be able to tell which button choice a user selected upon submission, even if we don't pass the info on to the destination page to auto-populate answers?)
• Include the small grayscale icon and Title text fields to denote "Messages from Firefox" [see attached example]
• Have option to display as "normal" (default), "half jimmy" (bold) or "full jimmy" (takeover) sized snippet [see attached example]

Adding @mwarther to this bug as a stakeholder who can help answer questions.
