so today I stumbled across this bug again. I clicked "Back" and got that
message. I have a nsHTP:5 log of it... I will attach what I believe to be the
relevant part in a second.
no, I have no way to reproduce this bug.
this is using build 2002090608 on win98.

this error usually indicates a leaked disk cache file. hopefully the HTTP log
will give us some clues about what URL might have resulted in the leak.

yup, getting 80520012 (NS_ERROR_FILE_NOT_FOUND) while reading from the disk
cache, and the document definitely exceeded 16k (putting it into a separate disk
cache file).
