This will give us the ability to use the isVisible() utility procedure in contexts where there is no global window object around. The current credit-card autofill code runs in such a context.

This gets us a version of isVisible() that will work when there isn't a fully populated window global around.
Also update the readme to reflect Fathom's new repo structure.
