Primary password requested too often, and entry window is seldom "on top"

Steps to reproduce:
Run Firefox normally, visit a site requiring authentication. Firefox displays a window requesting master password. password is entered. Firefox often asks a second time. Password is entered again. Login to visited website auto fills credentials as expected. Hours later visit another website requiring credentials. No auto fill occurs. The reason is because Firefox is requesting the master password again, and you can't see it because its not on top of any current open windows. So you have to minimize all windows to see it, and enter the password again.
Actual results:
As above, firefox often asks for the master password even though its the same run instance, just because I opened a new tab to visit another site needing credentials. The password entry box is also not on top of other windows, so all open windows need to be minimized to see it. In addition,after entering the password and dismissing the dialog, there will be a second identical dialog under it, requesting the password again.
Expected results:
After researching this problem, I found that the proposed solution was to remove the master password, shut down firefox, re-start, and put a new master password in. I expected that to change/fix the errant behavior, but it had no effect. If you can not fix the repeated request for password, please add the WINDOWS "always on top" attribute to the dialog box requesting passwords.
