Steps to reproduce:
Right clicked on main bar (just right of the search bar), and got the context menu of "MENU BAR; BOOKMARKS TOOLBAR ->; CUSTOMIZE TOOLBAR...", then followed BOOKMARKS TOOLBAR to "ALWAYS SHOW; ONLY SHOW ON NEW TAB; NEVER SHOW" and clicked on ONLY SHOW ON NEW TAB. I previously had it set to ALWAYS SHOW.
Actual results:
As I was not on a new page, the BOOKMARKS TOOLBAR disappeared. Then I clicked "+" to get a new tab. The BOOKMARKS TOOLBAR was NOT visible, yet it should be.
Expected results:
The BOOKSMARKS TOOLBAR should be visible on a new tab, and not visible on other tabs per the menu option being set.
Impressions: I decided that no real developer could have made such an obvious fault as to have an option that totally did not work, so I kept digging. A "new tab" can be either the "Firefox Home (default)" or a "Blank Page". I have it set to Blank Page. Set back to the default, it works as expected. Simple fix, a newtab can be either selection in the settings, the bookmarks toolbar needs to not be visible for either version of a newtab, per the settings, when the option "ONLY SHOW ON NEW TAB" is selected.
See also bug report 1654132 which is listed as an enhancement, but really looks like this feature. If newtab is modified by customization, then the bookmarks toolbar should still be visible or invisible per the menu option, that is to say, whatever comes up if you type "about:newtab" in the browser should make the TOOLBAR visible or not visible based on the menu options (ALWAYS = visible, ONLY SHOW ON NEW TAB = visible, NEVER = not visible).
