Steps to reproduce:

If I hide the title bar, icons for minimize, maximize, and close buttons are not shown and leaves a transparent zone.

I am using KDE git build, not sure if it's caused by the new default theme introduced in Plasma 5.23. Because on Plasma 5.22 the bug does not occur.

Thunderbird (93.0b4) has the same bug.

Im unable to replicate this since i don´t have that distro, but on ubuntu 20 and latest firefox release 92.0.1 i can see the buttons.

Does this issue occur in the latest nightly version of firefox? Here is a link from where you can download it: https://www.mozilla.org/en-US/firefox/channel/desktop/

I found other GTK applications also don't have the close icon, so this bug should be reported to upstream.

After deleting ~/.config/gtk-*, the close icon is back.
