Details

I launch Mozilla. there is a menu bar but no windows (this is on the Mac so that is possible). I try to open some windows. They appear in the "Window" menu but not on my screen. I can tab between them. I can close them with "Ctrl-W". I just can't see them.
One other thing: reinstalling Mozilla does not help. I think that there is a setting that it remembers in one of the JS files that is confusing it. Mozilla DID work for me for a long while before this started happening. And MailNews DOES work okay in the same session. But if I click on links nothing happens because they go to windows I can't see.
Paul, what OS version and Mozilla build ID are you reporting this bug against? Did this happen after you clicked the green maximize widget, perchance? Does the workaround on bug 167663's status whiteboard help you?

Paul, this sounds very much like bug 144027 ! At least the part with entries for new windows in the "Window" menu without having visible windows corresponding... Do you have third party themes installed? Try using a new(!) profile or deleting other themes and the "chrome" folder in your profile(!) directory. It might be necessary to delete more files from your profile (see the above bug), but usually *not* your prefs.js and personal data like your mails, so you can keep most of your profile.
No response for three months, description sounds very much like bug 144027. -> duplicate *** This bug has been marked as a duplicate of 144027 ***
