"It should be clearer on the revision that an uplift request form needs to be complete (or at least be called out by moz-phab); it isn't obvious that there's more steps than just moz-phab uplift"

Showing the form isn't viable; as submissions happen from the command line or Lando, while the form has to live in Phabricator.

I'm going to WONTFIX this in favour of bug 1982619.
