Steps to reproduce

1. Reach the payment form on www.llbean.ca
2. Fill in manually new CC info and submit the form

Expected result
Since the Card Name field does not exist, the CC Name place in the CC save doorhanger should be empty.

Actual result
"undefined" is displayed in the CC save doorhanger in the Card Name place.
