it's possible we're misunderstanding what populates the 'letter' placeholder - e.g., is it the domain URL or the page title?

In Focus, at least, the shortcut uses the first letter of the page title, not the domain name. This seems like a bad default because the shortcut for a cnn.com shortcut is the letter B because cnn.com’s page title is “Breaking News, Latest News and Videos | CNN”.
I suggest we use the first letter of the TLD+1 (which excludes subdomains, such as www. or news.) so a https://www.cnn.com/ shortcut would use the letter C if a favicon wasn’t available.
