During the toolbar redesign UX team revisited the ripple effects, and it was decided that:

The max height and width should be 32px * 32px as shown in this Figma link. Follow the same icon pressed colour as it is right now.
This is an attempt at fixing the existing buggy state of the pressed state of the icon buttons in the address bar. This was never defined, this is a carryover from the current address bar.
Yes, it should be applied to the current toolbar as well.
