Renumber all local patches sequentially (no gaps, no letter suffixes)
using git mv. Drop patches now carried upstream in 1.18.4 and clean
stale hg metadata headers on several preserved patches.

New local carries: 22-tor-scan-converter-polygon, 23-cff-subroutine-offset,
24-cff-fdselect-range-check, and 25-quartz-revert-to-pre-1.18.0 (the
composite revert that preserves the quartz-PDF stack).

Huh, at first I was assuming this was a new release, and was surprised at some of the local patches we still have present. But no, 1.18.4 is from a year+ ago, so recent-ish fixes aren't yet included.
