When Create an Attachment has been selected after submitting a bug, I next wee a
login document. Having completed that (yet again) I see the file submission
document which I complete correctly and submit.  I then see a login document
AGAIN, complete that and submit it only to receive the error document that no
file was selected - press back and try again.  This happens every time - so I
cannot submit an attachment as requested!!!!

Reproducible: Always

Error document 'no file submitted'

Expected Results:  
File submission accepted.

I was trying to submit an attachment for Bug 192597!!!!!!
