Steps to reproduce:

  1. Launch Firefox with title bar set to display
  2. Notice that the title bar is not displayed and neither are the window controls

Actual results:

Title bar is not displayed. Window controls are not displayed.

Expected results:

Title bar and window controls should be displayed.

Customizing the toolbar (right-click tab bar or menu bar) to turn off the title bar restores the window controls. Re-enabling the title bar does restore the title bar, but it and window controls are gone again the next time Firefox launches with title bar enabled.
