We already implement ITextProvider::GetSelection, which retrieves the caret if there is no selection. If there is a selection, the caret will be at either the start or the end of one of the the returned selection ranges. It's usually at the end, though it can be at the start if the user selected backwards. It's not currently possible to determine which. ITextProvider2::GetCaretRange solves this by providing "a zero-length text range at the location of the caret that belongs to the text-based control."
