Steps to reproduce:
Title bar and windows control missing when Firefox is opened maximized. The title bar and windows control return if Firefox is minimized then maximized again.
Bug declared fixed but still present in Firefox 147.

Can not reproduce the issue on neither Win10(19042.2965) / Win11 (26100.7462) using Firefox builds 147.0/148.0b2.

Starting Firefox with a new profile solves the problem.
Then, if I launch Firefox from the command line with firefox.exe -P and choose the former profile, the bug does not occur. However, if I set the former profile as default and relaunch Firefox from the taskbar shortcut, without going through profile manager, the bug occurs.
The taskbar shortcut simply leads to C:\Program Files\Mozilla Firefox\firefox.exe, with no window size option selected.
It might be worth emphasizing a couple points from one of my details (see above):
My specific environment: Windows 10 Home ver. 10.0.19045, with the taskbar locked at the bottom.
- My Windows 10 (Home) version is more recent than the 19042.2965 used in the test that failed to reproduce the bug.
- Note the with the taskbar locked at the bottom part -- might this be a relevant factor?
Also, when the bug manifests, do we know whether the Firefox title bar is actually absent or whether the window is too tall and pushes it offscreen?

This has been happening to me as well I have to right-click the menu bar area and select "Customize Toolbar" and then toggle the "Title Bar" box to restore it.

By "toggle" you mean change the value from what to what? Having "Title Bar" in the checked state does not work for me. How about the other factors I mentioned -- do you launch from a shortcut with "Run: Maximized" and is your Windows taskbar locked at screen's bottom? And you're on the same Windows 10 build?

I see. If that doesn't result in a "permanent" fix for you, it might be easier to press F11 twice after startup, which works (temporarily) for me and others with this issue.
How about your Windows taskbar -- locked at the bottom of the screen?
