There is no signal for how often page extraction fails/succeeds, or why. When the
assistant reports that it cannot read a page, the failure is not recorded,
so extraction problems surface only through manual reports and cannot be
ranked by frequency or impact.
URLs cannot be recorded, so the categorical failure reason has to carry the
diagnostic weight on its own; site-specific debugging will still require
manual reproduction.
