Need to add error states to the create monitor form. the Url needs to also turn into a pill on blur if the url is correct. There are some notes on the design to combined a few error messages.
- add error messages on URL
- add error message on monitor name
- add error message on prompt input if empty
- add "turn url into pill on blur" functionality.
