I have been using Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.4a)
Gecko/20030401 for a few months now, and have not had any problems so far. 
Recently, the download manager stopped working.  I cannot even open it.  The
program returns no error messages, it simply will not open.  I cannot tell you
how to reproduce the problem, because it just happened, and nothing I do makes
any difference.  I cannot save a web page or download any files.  My preferences
are still set to "open download manager" when "downloading a file."

Actual Results:  
nothing downloads or saves.

Expected Results:  
download or save what I wanted.
Sounds like a corrupted downloads.rdf file.
Find that file (it will be in your profile folder), delete it, restart Mozilla,
and you should be good to go.  
if deleting downloads.rdf doesn't help, try window | web development | 
javascript console and see what you have anything unusual there.

IMHO this bug could be closed. > 1 year no response from the reporter and an old
version.
