If I have an address book entry for Joe Smith with joesmith@domain.com, I can
later find duplicate entries for Joe automatically created in my address book.
It would appear that the cause of this is a different case of his email address:
JoeSmith@domain.com. I am not sure if this is further complicated by the
reversal of his full name and or the addition of quotes surrounding his name by
his emailer: i.e. Smith, Joe or "Smith, Joe"

Expected Results:  
If there is a case insensitive match on an email address, then no new address
book entry should be created, regardless of case of email address or proper name
of email sender/addressee.
