The File->Open dialog is missing icons on the "Go up a level" and "Go to home"
buttons.

Reproducible: Always
Steps to Reproduce:
1. Click on File -> Open or press Ctrl+o
2. Look to the right of the current path.

Actual Results:  
The buttons were blank.  Their tooltips appeared on mouseover and told me what
they did, but that was the only indication I had as to what the buttons were.

Expected Results:  
The buttons should have had the usual up-a-level and home icons.
