nsIStreamLoader::request is null in some error cases.  This happens if loading a
file:// URL that references a non-existant file.  We should fix nsStreamLoader
to store mRequest while it is calling OnStreamComplete.  Otherwise, our behavior
is inconsistent for consumers.
