When adding a new monitor to the display using nvidia nview, the menus continue
to appear on the original monitor. e.g. using laptop display first, then add a
CRT. Move firefox window to CRT. Click on menu bar and menu appears on LCD.
Closing all firefox windows and restarting fixes this behaviour.
In reverse, when deactivating a monitor, other applications move to the only
active monitor. Firefox appears on the taskbar but can't be coaxed to display on
the only remaining monitor. Closing Firefox from the task bar and restarting
fixes this.
