256215 - No text boxes for username/password in "Subscribe to Remote Calendar" or "Publish Selected Events" dialogs

I want to publish my calendar to a webdav server, and then subscribe to it from
other computers.  The server uses username and password authentication, but I
cannot enter my username and password.

Reproducible: Always
Steps to Reproduce:
1. Load Sunbird.
2. Click Tools | eitheroption to open the dialog box
3. Click Publish / OK depending on dialog box.

Actual Results:  
Sunbird freezes.

Expected Results:  
Open a prompt for username/password or complain that authentication failed.
