There's not much to describe. I've had several situations when Talkback
initiated its action, but it has always seemed to fail to send its reports. And
last time, it crashed. I would suggest that some logging/tracing information be
added to the tool to allow indentification and fixing of problems.

Look for workarounds at the related Mozilla Suite bug:
Bug 253750 Talkback doesn't launch anymore on crash
duping, as there is no Component:Talkback in Product:Firefox
