When creating a bookmark, if no custom icon is being provided by the site the
bookmark icon will default to a page with the corner turned down. Seemingly at
random, several times now I have surfed to google or one of my other bookmarks
that do have a custom bookmark icon and I suddenly see that other bookmarks on
my bookmark toolbar have acquired that pages icon. for example, the |G| icon
google supplies when you bookmark has now taken over a bookmark to a forum I
frequent that is in no way related or associated with google. This has happened
before, with a different icon being slapped onto a different bookmark to a
different site (this time a web comic icon was placed on a bookmark for
Weather.com) Only way of fixing this on my end I am aware of is deleting the
bookmark and recreating it. The only commonality I can think of is the wording
of both bookmark's names starts with a <

this is happening to me as well.  Also running XP and my bookmark icons (in the
folder and on the toolbar) change their icons randomly.  Some examples:

Appstate.edu now has the slashdot icon "/."

thehutchens.com has taken on the icon of adriansrojakpot.com

I don't see any similarities between the 2

there's no pattern to it that i can see. i've seen bookmarks on both the
bookmark list and the bookmark toolbar change to icons of different bookmarks
that the neither the address nor the bookmark name are even remotely similar.
there also doesn't seem to be any difference what you have in the bookmark name.
plain letter, numbers, symbols, it all appears to me (knowing as little about
software coding as i do) as completely random.

I can't pinpoint when this started happening to me (version #) but I don't think
it has always been this way (since earlier Firefox versions).  Anyone else have
an idea?

I've seen this since before 1.0

I've been able to reproduce it as follows:

put the following two bookmarks on the bookmark toolbar, 

http://www.osnews.com
https://internetbanking.aib.ie/hb1/roi/presign.jsp

but don't put them beside each other (ie add osnews one, then add something
else, then add aib one)

now, in the same tab, load the osnews site, load the aib site, load the osnews
site, load the osnews site, lather, rinse, repeat, and eventually the aib one
will take the osnews icon. if it doesn't seem to work, restart firefox and try
again. also try loading the aib one while osnews one is still loading the page.
there doesn't seem to be any hard and fast rules, but i've just reproduced it 10
times in a row by following the above instructions.

i think this should be marked as ALL instead of Windows XP, i'm experiencing
this on linux, Firefox/1.0.4

I have the same phenomenon too. It remains in beta 1.05. Icons of bookmarks in
the tab bar are arbitrarily replaced by others. The highest amount of identical
icons I've ever had is four. Bookmarks are affected either they have their own
icon or have no icon innately.

yea im having this problem too.like when i put a gmail bookmark there.then i put
like another bookmark without an icon.i click the one with the icon and then the
one without and then again and again.most likely it will change

a possible bug on 3.x is bug 411966
previous issues are invalid since favicon service and places have rewritten most code.
