I've written a test case to isolate the situations where we still don't get this right - it looks like we do for rule blocks, strings, and comments, but not for other sorts of blocks or parenthesized expressions. It's not (as far as I can tell) possible to determine with this sort of test case whether or not we get it right for [] constructs. The file should render with "T" and "R" columns identical. Blank white squares in the "T" column are the cases we currently get wrong.

I'm closing this bug; the only remaining issue that anyone knows about is bug 446226 and I don't think there's any value in keeping this open as a tracker. The patches went in long ago in other bugs.
