Only on Mac OSX.
Open Extension Manager. From Extension Manager, open Preferences for an extension that has one (I've confirmed this with Forecastfox and my own as-yet-unreleased extension). Then open Bookmarks Manager by selecting "Manage Bookmarks..." from Bookmarks menu. Nothing appears to happen, as Bookmarks Manager opens behind all other windows instead of on top.

Marking WFM, code must have changed in the 3+ years since this bug was opened, because the Bookmarks menu and all other menus except for "Firefox" go away when a dialog is open. Tried it with some windows, such as the preferences window, and the bookmarks organizer appears on top. Feel free to reopen if it's still a problem.
