Single signon raises two confirmation dialogs, then crashes

Details
If you respond to single sign-on's confirmation dialog by re-clicking the
form's submit button, Mozilla raises a second confirmation dialog. If you then
click No on one of the confirmation dialogs, the new page is displayed, but the
second confirmation dialog remains. If you click No on the remaining
confirmation dialog, Mozilla crashes with a NULL dereference.
I've set up a simple repro page at the listed URL.

If the single-signon's confirmation dialog were truly modal, you would not be
able to click on the form-submit button at the time that the single-signon
dialog was up. So this is a dup of bug 25684.
