Which version of Firefox are you running? I cannot reproduce with Mozilla/5.0 (Windows; U; Windows NT 5.1; en-US; rv:1.9a1) Gecko/20060125 Firefox/1.6a1.

Looks like an issue with the ATI HydraVision program - maybe try disabling it if possible?
Saw this today after activating Hydravision. Disabling "Enable dialog repositioning" and enabling "Remove HYDRAVISION titlebar buttons" in the Hydravision options seemed to work around the problem. I haven't seen this in any other apps, so perhaps Firefox does something unusual with its windows or dialogs? In any case, it's most likely ATI's bug.
