c.f. bug 312930, the context menu for design mode should be improved.

At the very least it should include the input/textarea items. I'm not sure whether we should include image or link items though, not all of them make sense, and we probably have access key conflicts too. Finally we have to decided what to do with the "This Frame" submenu, but my guess is that it's useless too.
