Go to a forum page, or any other page with a TEXTAREA, while a background page is still loading. When the background tab finishes loading, it will steal focus from the textarea and put the focus back into the content area. (This is easily reproducible on the MZ forum, and I believe it's a filed bug.) Here's the tricky part: time this so that you're in the middle of typing a word when focus gets stolen. Put focus back in the text area, and finish typing the word. The spellchecker will never realise that the word is spelled properly now, and will leave the red-dot underline underneath it no matter what you do (unless you delete the entire word and re-type it).

The focus-stealing bug exists only in embedding apps; see commentary on bug 306245. You could try cmd-tab in Fx but it won't be a true equiv since cmd-tab should send some notification that the app is becoming inactive....
BTW, this might be bug 339478 (it's relatively similar-sounding). Regardless, you should keep an eye on the tracking bug for spelling (bug 338999) and add relevant bugs to its dep list ;)
So, in BE if I switch tabs in the middle of a word, the word is marked misspelled, but when I switch back, the focus/cursor is right back where I left it, and when I complete the word, it loses its red line. So who knows.
This is WFM now (using my testcase for the evil focus-stealing bug, attachment 200323 [details]).
