Using:
export MOZ_DISABLE_IMAGE_OPTIMIZE=1
the problem goes away.
Pavlov provided a sample image which didn't look properly and i sent him a picture of it. The env var above fixed both my real images and his.
http://toolkit.crispen.org/formats/png.html isn't a perfect example (in the real case, I can never get the image to render, and actually it seems to have the same problem for a file that claims to be a gif), but the image on the left doesn't render when i load or reload the page. if i view just the image, it still doesn't render. if i reload the viewed image, it does.
In a browser w/ the env var, things "just work".
X server bug, if it goes away with disabling image optimization. Ubuntu dapper is fairly old.

An X server bug that old is probably old news; please reopen if that's not the case.
