Bug 339127 introduced a way for web pages to override the user's spell-checking preference (and disable spell checking); that bug also added a way to allow the user to override the page's decision (and apparently added the UI to Firefox as well, a toggle cm item).

Our textarea/input[type=text]/Midas (bug 348695) context menu needs to include this option as well.  In addition to allowing users to turn spellchecking back on, this option will also allow users to turn spellchecking off where needed (e.g., Bugzilla's patch-editing field, until Bugzilla revs to a version that turns it off by default).

Some sites (notably the rich text editor in vBulletin forums) are already disabling spellchecking, and users are already complaining to us.  (Note that in the vBulletin case, you'll have to fix bug 348695 before this fix is any good).
If we add the Cocoa spelling context menu to our context menu in textfields, it's possible (I guess) that we could link this function to the "Check Spelling as You Type" item in that menu, rather than creating a separate menu item as Fx has done.
We dont explicitly disable anything within vBulletin, its to do with the way the designMode iframe is created. I've since added code to enable it by default and there is bug #357207
