I guess this is fallout from Bug 233853. Also applies to Trunk.
1. Type a single-word keyword into the Location Bar and hit Enter
2. Go back to the Location bar and start typing the same keyword
3. Select http://keyword/ from the Autocomplete list and hit Enter
Actual Result:
Sent to keyword.com, regardless of keyword.url result
Expected Result:
Sent to result of keyword.url for the keyword
Ideally http://keyword/ shouldn't be shown as-is in the Autocomplete list since the expected result isn't clear.

Reporter, do you still see this problem with the latest Firefox 2? If not, can you please close this bug as WORKSFORME. Thanks!

I see this in the latest nightly trunk.
I tested two different situations:
1. Type in location bar "keyword" > taken to google search for "keyword" > focus on location bar > type "keyword" or press down results in "http://keyword/" in location bar dropdown > select this and press enter > taken to keyword.com
2. Type in location bar "dog" > taken to google search for "dog" > focus on location bar > type "dog" or press down results in "http://dog/" in location bar dropdown > select this and press enter > taken to dog.com
Seems to my like keyword history should be saved as the entered keyword. "http://" should not be appended to the keyword.

This isn't really the same issue. It seems like the problem is that after entering a keyword and pressing enter, it puts that entry in your history as http://keyword/. (At least in Firefox 2.) This isn't even related to smart keyword bookmark searches.
A second problem arises when you type keyword and select http://keyword/. The location bar code doesn't assume it's a keyword search (neither keyworded bookmark nor generic google search) because it starts with http://, so it tells the back-end to load http://keyword/, but that site doesn't exist, so it guesses you actually meant http://keyword.com/
