375824 - Centralize Thebes startup and shutdown

As discussed on IRC, more or less... We should do all Thebes global init and shutdown via gfxPlatform::Init and Shutdown. Currently gfxPlatform::Shutdown is called from gfx/src/thebes/nsThebesGfxFactory.cpp, so I put the ::Init call there too.
