inserting an image at beginning of file disables much UI

Details
Today when I was playing with D&D of images from the Finder into Composer, I
found that if I inserted an image (via d&d or image dialog) as the first item in
the document, most of the UI became disabled (such as Quit under File menu).
Even if you change the caret/selection or if you type additional text etc, the
commands don't seem to get enabled.
Inserting an image later (after having typed) seems to work fine.
