Alright so we all know how the devs feels about adding the 'Clean Up' button back to the download manager so this is what I propose.  Since they say that people lose their files then so be it, don't add the 'Clean Up' button back but add some code to remove the history for a file, if that file has been deleted. Problem solved for everyone, everyone is happy!

Peter mentioned something similar at bug 392446 comment 1 and initially I thought it was a great idea. But then I remembered that the items in the download history also store the URL the file was download from, so perhaps it's not so great to throw this information away. (user may want to download the file again, or at least find what site the file was downloaded from, etc).
