pkix_pl_OcspRequest_Create can leave some members uninitialized

I was reading pkix_pl_OcspRequest_Create

For example, if the function arrives at
  (locError == SEC_ERROR_CERT_BAD_ACCESS_LOCATION)
it will jump to cleanup, and ocspRequest->certList will remain uninitialized.
