Somewhere between alpha5 and alpha6. Was working fine till alpha5; is broken since alpha6. I didn't test nightlies released between these 2 milestones.
I tested nightlies released between a5 and a6, and I have been able to narrow the window:

Works till:

Mozilla/5.0 (X11; U; Linux i686; en-US; rv:1.9a6pre) Gecko/20070604 Minefield/3.0a6pre

Broken since:

Mozilla/5.0 (X11; U; Linux i686; en-US; rv:1.9a6pre) Gecko/20070605 Minefield/3.0a6pre

Looks like bug 329846 has been checked in in this timeframe. It could be the culprit.
That's most likely a theme-specific bug. :(

Definitely theme specific. It works in every theme I have over here.
Yeah, sounds like something similar to bug 388131.  Possibly similar enough to be a duplicate...  Worth reporting to the author of the GTK theme in question, too.

Definitely similar enough to the problem described over in 388131, duping. I'm wondering, Boris, did you see some reason not to dupe this over there when you noticed the similarity?
