Details

Currently when an extension installed as a system extension gets updated, the update is in the user profile dir, even though the original extension was in the extensions dir under bin. This creates some weird situations like:

original extension was hidden, updated extension is not.
Uninstalling extension backlevels to system installed extension.

If write access is available to the bin/extensions directory, the system extension should be updated in place.
What do you mean by "system extension"? If you mean something with the "system" flag set in RDF, that's supposed to mean we don't update it at all, I believe.
