408732 - Add Bookmark dialog remembers first responder (default focus)

I noticed that the Add Bookmark dialog remembers its first responder.  I don't like this.  Do you?

Steps to reproduce:

Turn FKA on
In Camino, press Command-D to open the Add Bookmark dialog
Note which element has focus
Tab to another element
Hit Esc to close the dialog
Press command-D to open the dialog again

Observed: Whatever element you tabbed to in the dialog's previous life is remembered as the initial first responder in the newborn dialog.

Expected: Focus should reset to the bookmark title text field each time

We can do this as a band-aid, but we should fix all of bug 408734 instead.  That bug is much more serious.
